After 41 years of dedicated practice, Dr. Cornatzer has retired and Bismarck Dermatology Center closed May 1, 2026.​ We are deeply grateful for the trust and support our patients have shown over the years. It has been our privilege to care for this community.

If you need your medical records sent to another provider, a completed Release of Information form is required. If you do not have the form that was mailed to you, you can access it HERE. Completed forms may be mailed to our current mailing address or sent to clinicop@bismarckderm.com.
We will continue handling record requests through July. After that time, our records custodian will handle all requests for records and that information will be added to our website in late July. Please be assured that your medical records are secure and will continue to be available for years to come.

We have already received thousands of record requests, so processing will take time. We appreciate your patience and want to assure you that all requests will be processed.

If you would like to make a payment by credit card or HSA card, you can mail your payment information to our current address, as listed on your statement. If you don't have a current statement, please be sure to include the patient's name, date of birth, amount to be paid, and your card information.
